Pricing & Membership
Membership is $129 per month for Practice or $249 per month for Concierge. Medications are billed separately at member rates. With Practice Membership, Glucose Foundation comes to $179 per month, Energy & Recovery to $457, and Testosterone Therapy to $253.
Practice Membership covers your initial video visit with a physician, three async clinical consultations per year, and care coordinator support with a 24-hour response. Concierge raises async consultations to eight per year and adds two sync video consultations. Both tiers give you access to the full prescription catalog at member rates.
Concierge is for patients who want more physician time. At $249, it raises async consultations from three to eight and adds two sync video consultations per year. The default tier is Practice.
Separating membership from medication cost lets you add or change protocols without renegotiating a bundle. Practice Membership at $129 covers physician oversight and consultations. Each protocol's medications are billed monthly at member rates. Adding a second protocol means adding the medications, not a new membership.
No. Prologue is a cash-pay practice. Itemized receipts are available for HSA and FSA reimbursement claims.
No. Membership is month to month. Cancel from your portal at any time.
Yes. Annual prepay is available after your first protocol month and is 17 percent off the monthly rate. Practice Membership is $1,290 per year and Concierge is $2,490 per year.
Your Physician & Care
Prologue's physicians are licensed in the states where they see patients, and you're matched with one licensed where you live.
For video visits, on request. By default a consultation is matched to a physician based on availability and the time you choose. If you want continuity with one physician for your video visits, ask, and the practice will book them when their schedule allows. Async consultations are reviewed by the next available Prologue physician.
Through your consultations: async clinical consultations are included in both memberships, and video visits where your plan includes them. For anything between consultations, write to the care team. Clinical questions get a reply within 24 hours.
Every member's first visit is a video call with a physician. Ongoing care happens through async clinical consultations, three per year, with additional consultations available. Additional consultations: pricing confirmed at enrollment. Concierge Membership includes two sync video consultations per year on top of the intake. Testosterone Therapy patients on either tier receive the four quarterly sync visits required for Schedule III monitoring at no additional fee.
No. Prologue is not an emergency service. For urgent medical issues, contact your primary care physician or call 911 or local emergency services. Clinical questions get a reply within 24 hours.
Your Protocol & Medications
Prescriptions are compounded by a licensed compounding pharmacy partner. Medications ship to your door at the prescribed cadence.
Most Prologue medications are built on active ingredients the FDA has approved in commercial form, including metformin, acarbose, testosterone, anastrozole, bremelanotide (PT-141), tesamorelin, methylene blue, and vitamin B12. Ours are compounded by a licensed pharmacy, and compounded medications are not FDA-approved or reviewed by the FDA for safety, effectiveness, or quality. Your physician prescribes most of them off-label for longevity. NAD+ and sermorelin have no FDA-approved commercial form on the market. Each protocol and medication page lists the regulatory status of what it contains.
A compounding pharmacy prepares a medication to a specific formulation rather than dispensing a mass-manufactured commercial product. Compounded preparations are not FDA-approved as compounded products. Each batch is tested for potency, sterility, and endotoxin levels before shipment.
Standard shipping for oral medications. Cold-chain shipping for injectables.
Yes. After your first protocol month, you can ask your physician whether a second protocol is clinically appropriate. The membership and physician relationship continue. You add only the second protocol's medication cost.
Yes. Fifteen products are available at member rates, and nine of them are prescribed individually rather than as part of a protocol. Your physician can also prescribe a protocol medication on its own when that fits your situation. The full catalog lists everything with member pricing.
Protocol adjustments happen as your response shows up. If the dose or cadence changes, your next shipment reflects the change. There is no additional fee for protocol adjustments within your existing protocol.
Testosterone & Controlled Substances
Testosterone cypionate is a Schedule III controlled substance. DEA regulations require an initial video consultation with the prescribing physician before treatment begins, and quarterly video visits during ongoing therapy. These visits are included in Practice Membership at no additional fee.
The initial consultation and quarterly monitoring visits require synchronous video. Between those, your care continues through async clinical consultations like any other patient's.
Yes. Bring your most recent lab work (within the last six months) and your current prescription. Your physician reviews, confirms the protocol, and continues your treatment. The initial video consultation is required to establish the patient-physician relationship.
No. Testosterone Therapy at Prologue requires confirmation of low or low-normal serum testosterone from recent lab work before prescribing. If the results fall outside the indication range, therapy is not prescribed and you are not charged.
Cancellation, Refunds & HSA/FSA
Yes. Cancel from your portal at any time. There is no cancellation fee, and your membership stays active through the end of the month you've paid for.
Nothing is charged when you join. Billing begins only after a physician has approved your plan. If they decide treatment isn't appropriate for you, you are not charged.
Membership fees aren't refunded for partial months. When you cancel, your membership stays active through the end of the period you've paid for. Because nothing is charged until a physician approves your plan, there is nothing to refund if treatment isn't approved.
Itemized receipts are available for HSA and FSA reimbursement claims.
Yes. Pause from your portal and your protocol holds. Your prescriptions stay on file, your consultations pause, and nothing ships until you resume.
Privacy & Data
We do not sell your health data. Prologue does not sell or share patient data with third parties. Your records are used to provide your care and are shared only as the Privacy Policy describes.
You and the physicians treating you. Operational staff has access to administrative records but not clinical data. Your records are not shared outside the practice without your written authorization.

State Availability & Getting Started
Yes. Prologue is available in all 50 states, and you're matched with a physician licensed where you live.
Join the practice. Choose a membership. Nothing is charged until a physician has approved your plan.
You complete a fifteen-minute clinical intake, and a Prologue physician reviews it with you on a video call. If a protocol is right for you, it is prescribed and your first shipment goes out. If the physician decides treatment isn't appropriate, you are not charged.
